XLIF - eXtreme Lateral Interbody Fusion

Matthew Tormenti MD, FAANSBy Matthew J Tormenti, M.D., FACS, FAANS

XLIF (eXtreme Lateral Interbody Fusion) is a newer minimally invasive procedure which is performed through the side of the body. The procedure is used to treat spinal disorders and to reduce long-term back or leg pain. As with most procedures, XLIF is not recommended unless the patient’s condition has not responded to other treatments, such as steroid injections, physical therapy and pain medication.

XLIF is a type of interbody fusion. This means that it is a fusion surgery where a disc is removed and replaced with an implant containing a bone graft. The bone graft facilitates the fusion of two vertebrae through the disc space. Other types of interbody fusion commonly used to treat lumbar spinal problems include ALIF, PLIF and TLIF.

The primary difference between XLIF and traditional procedures is that the surgeon accesses the space between each spinal disc from the patient's side, rather than from the front or back. This approach has the effect of sparing major back muscles, bones and ligaments. Occasionally, patients may also need screws and rods placed to support the XLIF. This is usually done through a separate incision in the back and done in a minimally invasive/disruptive fashion.
Potential Advantages of XLIF may include:

Conditions Treated

XLIF may be recommended to treat the following lumbar or lower spine disorders:

Procedure

The XLIF procedure is performed under a general anesthestic, so you will be asleep during the surgery. Steps of the surgery include:

Recovery
Because XLIF is less disruptive than conventional surgery, most patients can walk the evening after surgery and are discharged from the hospital the next day.

Risks
All surgery involves potential risks. Possible risks and complications include:

As with any procedure, the surgeon will fully discuss the potential risks and benefits with the patient.
Dr. Matthew Tormenti has specialized training in the XLIF procedure, and is fully qualified to perform this surgery where appropriate and indicated. He has authored several papers and given several talks regarding the procedure.
